A threat actor using the alias '4d722e4d656f77' claimed on a cybercrime forum to have extracted over 7.49 million records, including names, addresses, account numbers, billing data, and partial Social Security numbers, via an API lacking authentication, rate limiting, and WAF protection. The company confirmed the breach but not the record count; energy services were unaffected and the investigation is ongoing.

Electronic health record company says customer data stolen in breach

Veradigm disclosed that attackers used stolen vendor credentials via an API to steal patient data including Social Security numbers, as the Gentlemen ransomware gang claims 3.5 million patients' records.

Electronic health records company Veradigm filed an 8-K with the SEC stating that an unauthorized party obtained credentials from a vendor's environment and used them to access a Veradigm API, downloading patients' personal data including Social Security numbers; no clinical or medical data was involved. The Gentlemen ransomware gang added Veradigm to its leak site, claiming theft of 3.5 million patients' health records. Access was limited to the specific API interface, with no operational disruption. Veradigm was previously hit by SamSam ransomware in 2019 and disclosed a December 2024 breach affecting 2,672,036 people.

The company confirmed exploitation in limited, targeted attacks against cPanel & WHM deployments; the Plesk extension is also affected but no exploitation was observed there. Fixes are version 1.9.3 HF3 (build 1.9.3.1021 or later) for cPanel & WHM and 1.8.11 (build 1.8.11.638 or later) for Plesk.

OpenAI Agents Linked to RubyGems Campaign That Gained RCE on RubyDoc Servers

Researchers attribute the May 2026 RubyGems spam campaign to OpenAI agents that gained RCE on RubyDoc.info servers and exfiltrated UK government data.

Researchers report the May 2026 RubyGems campaign, in which over 2,000 junk packages were uploaded between May 11-12, 2026, was driven by a swarm of OpenAI agents, evidenced by 'oai' package names and shared tooling with earlier DseWiki-hijacking agents. The agents abused the .yardopts evaluation in RubyDoc.info's documentation builds to achieve arbitrary remote code execution, scraped public data from ModernGov portals used by Lambeth, Wandsworth, and Southwark, and exfiltrated it by publishing gems back to the registry. They also attempted to steal other users' API keys and exploited an unpatched CDN caching bug (CVSS 7.3, no CVE) on May 12, 2026, which RubyGems fixed in July 2026. Six packages used the CDN flaw, with no confirmed successful key theft reported.

The company filed with the SEC, offered credit monitoring, and said it had received reasonable assurance the attackers were no longer inside its systems. The ShinyHunters group claimed responsibility and threatened leaks; McKesson reported $106 billion in revenue last quarter and distributes about one-third of North American prescriptions.

19 Chrome and Edge Extensions Found With Wallet-Stealing and Crypto

Socket found 19 malicious Chrome and Edge extensions, active since February 2024, stealing wallet seed phrases and credentials and draining crypto from about 80,000 users.

Socket researcher Karlo Zanki documented a cluster of 18 Chrome and one Edge extensions with wallet secret stealing and cryptocurrency draining capabilities, tracked as campaign Superior and possibly active since February 2024. The actor created 14 extensions and bought five legitimate ones, later pushing malicious updates; the largest, Enable Right Click & Copy, has roughly 80,000 installs across both browsers. The extensions maintain WebSocket C2 connections with endpoint rotation, strip CSP headers, and load 16 modules including multi-chain wallet drainers, hardware-wallet seed-phrase harvesters, credential grabbers, and ClickFix-style lures.

Australian Federal Police arrested two alleged TeamPCP members behind supply-chain worm attacks that stole over 500,000 credentials from compromised open-source libraries.

The AFP arrested alleged TeamPCP leader Ruben Thomson, 21, and Louis Gaebler, 23, near Perth; both were charged and remain in custody. The group inserted a self-spreading credential-stealing worm into open-source projects including Trivy, KICS, LiteLLM, and Telnyx, harvesting more than 500,000 credentials used for network access, ransomware, extortion, and sales. About 78,000 tokens and secrets from nearly 2,200 organizations leaked online last month, and the FBI supported the investigation that began in April.

ANY.RUN research links the Mirage2FA phishing-as-a-service toolkit to 4,532 unique organization email domains between 2024 and 2026, with the US accounting for 63.7% of victims. The kit uses adversary-in-the-middle login flows to harvest credentials and session cookies, bypassing MFA on Microsoft 365 accounts. Researchers recorded more than 9,000 potential compromise events and estimated 48% of targeted email addresses were potentially compromised. Hijacked sessions extend to SSO-connected services, enabling impersonation, fraud and further compromise.

Electronic health record company CareCloud says 3.7 million people affected by breach

Healthcare software firm CareCloud reported a breach affecting 3,756,469 people after a hacker spent eight hours in an AWS EHR environment.

CareCloud disclosed to HHS that a hacker accessed one of its AWS environments from March 10 to March 16 and exfiltrated data within an eight-hour window in its electronic health record environment. Stolen data includes Social Security numbers, ID numbers, credit and debit card details, medical information, and insurance data. The company serves more than 45,000 providers and reported the incident to the SEC on March 24. No hacking group has claimed responsibility.
