Patch Tuesday - September 2026

Microsoft's September 2026 Patch Tuesday fixes 999 CVEs, a record, with two zero-day privilege escalation flaws already exploited in the wild.

Microsoft published 974 own-product vulnerabilities plus 25 non-Microsoft CVEs, totaling 999 — the most CVEs Microsoft has ever released in a single day. Two flaws are exploited in the wild: CVE-2026-85880, an out-of-bounds write in Windows ALPC granting SYSTEM privileges, and CVE-2026-81963, an improper link resolution flaw in the Windows Update Stack also leading to SYSTEM. Chrome's V8 zero-day CVE-2026-85046 was patched in Edge on September 2, but Microsoft had not published a corresponding advisory, leaving uncertainty about other Chromium fixes in Edge. October 14 lifecycle changes end servicing for Windows 11 24H2 Home/Pro, Office 2021, and Exchange Server 2016/2019.

Halo-record: Open-source audit trails for AI agents

Developer Brian Kuan released halo-record, an open-source Python package creating tamper-evident, hash-chained audit logs of AI agent actions.

Halo-record is a roughly 5,300-line Python package with no runtime dependencies that records agent tool calls, model calls, data access and approvals into an append-only, hash-chained log that customers can verify without vendor trust. Adapters ingest records from OpenTelemetry spans, LangChain, MCP servers and gateway logs, with secret and PII values auto-redacted. The author plans to fund the work through a hosted witness service that stores the record count and head hash to prove completeness, citing mandates like AIUC-1, the EU AI Act, and insurers. The article cites the July Hugging Face intrusion, where an autonomous agent took roughly 17,600 actions over five days and manual reconstruction of its activity was impractical.

Doppler's secrets management platform centralizes credentials for engineers, CI/CD pipelines, MCP servers, and AI agents with runtime injection and dynamic secrets.

Doppler stores API keys, database URLs, tokens, and certificates in a single control plane and injects them at runtime, replacing .env files for human and machine identities including AI agents. It supports OIDC with short-lived identity tokens for Azure, AWS, and GCP, dynamic secrets scoped and time-boxed to single sessions, SCIM provisioning, 50+ integrations, and an MCP server that lets agents request configurations natively. Permissions are enforced at each layer so raw secrets stay out of logs, prompts, and model context, with versioning, rollback, SIEM log forwarding, and cloud or on-prem deployment.

Context Engineering Inside the Harness: 4 Mechanisms That Beat Context Overflow and Goal Loss on Long-Horizon Tasks

Survey of four harness mechanisms—context budgeting, compaction, todo-state, and memory—that keep long-horizon LLM agents on task across 200+ tool calls.

The article details how agent harnesses, not larger context windows, solve context overflow and goal loss on long-horizon tasks, citing Chroma's Context Rot report showing 18 LLMs (GPT-4.1, Claude 4, Gemini 2.5, Qwen3) degrade on long inputs. Concrete implementations include LangChain Deep Agents offloading tool responses over 20,000 tokens to the filesystem and truncating old tool calls at 85% window usage, and Claude Code capping auto memory at 25KB while re-reading the 5 most recently modified files after compaction. OpenAI's Responses API now offers server-side compaction via context_management with a standalone /responses/compact endpoint, which Codex uses for long-running coding tasks. TerminalFix Uses Fake Cloudflare CAPTCHAs to Deploy Reverse

Microsoft details TerminalFix, a ClickFix variant using fake Cloudflare CAPTCHAs to make users run PowerShell payloads with reverse-tunnel implants.

Microsoft disclosed TerminalFix campaigns that direct users to run commands in Windows Terminal or PowerShell instead of the Run dialog, increasing the chance multi-line scripts execute. Compromised websites serve fake Cloudflare CAPTCHA verifications; the resulting PowerShell command downloads a ZIP with legitimate LockScreenContentServer.exe and rogue dui70.dll for DLL sideloading, retrieves payloads hidden in PNG images from attacker domains, persists via Registry Run keys and scheduled tasks, and performs domain trust and Active Directory reconnaissance. A Python-based reverse-tunnel implant (client.py) tunnels arbitrary TCP traffic over an encrypted WebSocket to gitnow[.]dev:443, giving attackers network-level proxy access that could enable privilege escalation, security-tool tampering, data theft or ransomware. Microsoft recommends AppLocker restrictions, PowerShell script block logging and ClickFix awareness training.
