The Top 4 Threats We Found by Investigating Every Alert for a Quarter

Prophet Security's quarterly threat report finds identity attacks made up roughly half of confirmed malicious activity, with stolen authenticated sessions bypassing conditional access controls.

Between May 1 and July 31, 2026, Prophet Security investigated every alert in customer environments immediately on arrival; about 7% of completed investigations were confirmed malicious. Direct account/session attacks made up roughly 18% of confirmed activity, with replayed authenticated sessions bypassing conditional access since no re-authentication occurs. Infostealer activity affected about a quarter of investigated organizations, mostly delivered via browsers through compromised websites, malicious ads, sponsored search results and ClickFix fake CAPTCHA gates, with payloads including Lumma Stealer, Vidar, HijackLoader and AsyncRAT. Credential phishing was the largest category at about 28%, with adversary-in-the-middle relays revealed by seconds-long MFA approvals from distant countries and MFA fatigue attacks exploiting automatic unlock policies.

Key Reasons Why Identity Fabric Matters in 2026

Identity sprawl and unowned machine identities leave enterprise access unobserved at runtime; identity fabrics aim to close the gap between policy intent and execution.

This sponsored explainer describes identity fabric as an architectural approach connecting identity providers, governance systems, applications, and infrastructure into one observable layer that compares designed access intent with runtime execution. It argues identity sprawl across SaaS, APIs, and cloud workloads, plus unmanaged non-human identities (service accounts, bots, workloads, API keys), leaves overprivileged, dormant, and unowned machine identities unmonitored. IdP-only monitoring misses application-layer attacks, and the piece advocates behavioral visibility and lifecycle governance for secrets and machine identities.

Register Tokens for Bounded-State Reasoning in Diffusion Language Models

Register tokens let diffusion language models like LLaDA and Dream carry reasoning state across cleared chunks, gaining up to 19.5 points on code.

Researchers propose register tokens: dedicated fixed-position tokens whose continuous hidden states are trained to carry reasoning progress across generation chunks in masked diffusion language models. After decoding and clearing a chunk, the model continues from the prompt and the carried register state instead of retaining earlier text. On LLaDA and Dream, registers outperform discrete-text carry on every benchmark, with gains up to 8.5 points on math and 19.5 points on code. Registers are especially effective for bounded code generation and can be further refined with reinforcement learning on long-horizon reasoning tasks.

AI agent authorization risks remain a gap in new NIST-CISA token security guidance

NIST and CISA release IR 8587 guidance on securing signed tokens, but AI agent authorization and delegation risks remain out of scope.

NIST, with CISA support, published 'Protecting Tokens and Assertions from Forgery, Theft, and Misuse' (NIST IR 8587), recommending continuous monitoring and tighter token lifecycle controls for SSO and API access. The guidance does not yet fully address AI agent identity, delegation chains, or prompt injection steering agents with valid tokens, and NIST says new or expanded standards are needed. Experts recommend treating AI agents as low-trust non-human identities, maintaining agent inventories, expiring credentials after task completion, and requiring human approval for high-risk actions. The report references shared-signal mechanisms like CAEP and RISC, and follows a May incident where a CISA contractor GitHub repository exposed AWS and GitHub tokens.

FreeIPA Flaw Chain Lets Anonymous Clients Create Reusable Administrator Credentials

Critical FreeIPA flaw chain (CVE-2026-76578, CVSS 9.8) lets anonymous clients create reusable admin credentials on default installs; fixed in 4.13.4.

Red Hat warns that chaining FreeIPA's CVE-2026-76578 (CVSS 9.8) with a 389 Directory Server flaw (CVE-2026-76560, CVSS 7.5) lets an anonymous client write a Kerberos identity into the directory and land in the administrators group on default installations; Red Hat reproduced the chain twice, including on a machine with no access at all. FreeIPA 4.13.4 fixes both flaws, and fourteen 389-ds advisories shipped September 8, with RHSA-2026:64785 rated critical. A separate flaw, CVE-2026-79678 (CVSS 8.1), allows reading the server's environment variables one at a time via a Python eval() in idp-add, potentially exposing Directory Manager and administrator passwords in container installs. No real-world exploitation has been reported.

Water sector passes, government sector fails attempts to spot and halt simulated CISA attack

CISA red teamers compromised both a government and a water organization; water defenders detected and contained the simulated attack, government defenders did not.

CISA's red team gained initial access, elevated domain privileges, and lateral movement into sensitive business systems and cloud resources at an unnamed government organization, whose SOC ignored low- and medium-severity EDR alerts buried under thousands of false positives. A water organization's SOC quarantined phishing-compromised workstations within 2, 10, and 20 minutes, and later detected and isolated intrusions reaching the OT DMZ bastion host. Both organizations underestimated cloud risk, lacked Microsoft Conditional Access for workload identities, and had no process to revoke compromised access and refresh tokens. ResidualAuth: What Authorization State Must Language Agents Preserve under Revocable Delegation?

Formalizes residual authorization state language agents must preserve under revocable delegation; token-budget summaries mostly fail while hard gates stop unauthorized effects.

The paper shows two authorization histories with identical current permissions can require opposite decisions after the same direct-edge revocation, formalizing the needed information as residual authorization state. Exponentially many future-distinct states can share one transitive closure, with exact or tight asymptotic bounds on the state an exact monitor requires. Across four open-weight models, fixed 256-token summaries solved at most 2 of 16 paired episodes while authenticated current-query reads solved 15-16 of 16. A hard effect gate reduced eight observed unauthorized effects to zero without changing preceding attempts.

The Machine With Many Faces: Post-Exploitation Identity Misuse in SPIFFE/SPIRE

Unit 42 demonstrates that root access on a Kubernetes node lets attackers spoof SPIFFE/SPIRE attestation and harvest co-located workloads' SVIDs.

It also criticizes frontier safety filters for refusing vulnerability research tasks and calls for models that support exploitability testing in CI/CD pipelines.

The modern attack chain: Rethinking Google Workspace security in the age of AI

Analysis of Vercel and Composio breaches shows stolen OAuth tokens, not phishing email, now open Google Workspace attacks — a path authorized AI agents follow.

The author analyzes the Vercel and Composio breaches as the same OAuth-first attack chain run twice: a stolen OAuth token, obtained via a compromised supplier, becomes the entry point rather than email. These tokens survive password resets, are hard to observe, and let attackers read Gmail and Drive data, take over accounts, and pivot laterally using stored credentials and password-reset magic links. The piece warns that authorized AI agents with overbroad OAuth grants can unintentionally traverse the same path — accessing inboxes, reading sensitive content, and exfiltrating data downstream — without any malicious actor or compromised credential.
