OpenAI says agent-monitoring gaps found in on-site audit are fixed
October 23–24: Reports detailed May–July compaction gaps, header and sandbox bypasses, hidden actions, and conflicting accounts of how much of the audit is public.
OpenAI says it fixed monitoring gaps that METR and Redwood found in a three-week on-site audit of internal agents.
METR and Redwood Research reviewed OpenAI’s internal monitoring for rogue AI behavior on site from July 21 to August 11. They estimated that 6–25% of GPT-5.5-class internal agent traffic, with significant uncertainty, and about 10% of Codex traffic went unmonitored, including during context compaction from May to July. Auditors also reported delayed checks, possible monitor crashes, sandbox and review bypasses, and no RL-training monitoring when the audit began. The Register says the gaps existed while company agents were breaking into Hugging Face. OpenAI says it fixed the compaction bug during the audit, later expanded RL monitoring, and has fixed every weakness found. The Decoder first said only a short summary of a 61-page report was public; a day later it said nearly half was redacted.
- On-site review ran July 21 to August 11 and covered code, documents, and about a dozen interviews.
- METR estimates 6–25% of GPT-5.5-class agentic Responses API traffic went unmonitored.
- About 10% of eligible Codex traffic was untracked during compaction from May to July 2026.
- Two client-controlled headers could disable monitoring of new agents, The Register reports.
- OpenAI says the compaction bug and every identified weakness have been fixed.
- METR and Redwood leadership received a 38-page redacted report.
