CISA advisory AA26-237A documents two simultaneous red team assessments. Organization A (Government Services sector) missed the intrusion entirely, as default credentials on a web app, ADCS ESC1 abuse, and thousands of false-positive alerts let the red team reach sensitive business systems and read SOC email. Organization B (Water/Wastewater sector) detected, isolated, and reimaged hosts quickly, but both orgs lacked Conditional Access for workload identities, and B still exposed DCSync, Golden Ticket, and OT network attack paths.

CISA Updates Insider Threat Guide With New Mitigation Advice

CISA updated its Insider Threat Mitigation Guide on September 9 with new case studies and guidance on hybrid work, AI and employee separations.

CISA published a revision of its Insider Threat Mitigation Guide, first issued in 2020, adding case studies, statistics and guidance for security, HR and leadership audiences. New material covers hybrid and remote work changes to physical and digital access, AI used to manipulate or deceive, access control, visitor screening and adverse employee separations. The agency framed the update around growing insider threat impact on critical infrastructure and pointed to preparedness resources for organizations without existing programs.

Water sector passes, government sector fails attempts to spot and halt simulated CISA attack

CISA red teamers compromised both a government and a water organization; water defenders detected and contained the simulated attack, government defenders did not.

CISA's red team gained initial access, elevated domain privileges, and lateral movement into sensitive business systems and cloud resources at an unnamed government organization, whose SOC ignored low- and medium-severity EDR alerts buried under thousands of false positives. A water organization's SOC quarantined phishing-compromised workstations within 2, 10, and 20 minutes, and later detected and isolated intrusions reaching the OT DMZ bastion host. Both organizations underestimated cloud risk, lacked Microsoft Conditional Access for workload identities, and had no process to revoke compromised access and refresh tokens. This is one of CISA's rare public red-team reports since 2023.

CISA red teams fully compromised two critical infrastructure orgs with similar tradecraft; one detected nothing, the other contained phishing within minutes.

CISA advisory AA26-237A, 'A Tale of Two SOCs,' describes two simultaneous red team assessments of a Government Services organization and a Water and Wastewater Systems organization, both compromised to the domain level. Organization A never detected the intrusion, enabled by default credentials, a misconfigured AD CS template (ESC1), cleartext credentials, non-expiring static AWS keys, and over-permissioned Entra ID applications. Organization B detected and isolated phishing within 2-20 minutes, though the red team still reached krbtgt via DCSync from cleartext SCCM credentials. CISA attributes the outcome gap to people and processes rather than the security tools themselves.

CISA's Logging Reference Architecture (LRA), released in August 2026, helps US federal civilian agencies satisfy logging requirements in OMB Memorandum M-26-14 and explicitly encourages critical infrastructure operators to use it as a benchmark. The framework is organized around continuous event monitoring and threat hunting, investigation, response, and forensics, with a federal baseline of six months searchable and one year retrievable logs. Agencies must submit Agency Logging Plans within 90 days and work toward Advanced maturity within 320 days; the guidance also treats AI outputs as derived data requiring human review and preserved metadata.

CISA advisory: seven flaws in CareCam CM2507 IP cameras enable unauthenticated live video access, privileged ONVIF control, credential recovery, and code execution.

CISA advisory ICSA-26-258-08 discloses seven vulnerabilities in CareCam HMT.CM2507 IP cameras running firmware v251211.1507. Issues include missing authentication for network video streaming (CVE-2026-88259, CVSS 3.1 7.5), an empty password on a privileged ONVIF account (CVE-2026-84398), weak legacy hashing of the root password (CVE-2026-85497, CVSS 4.0 9.3 critical), and cleartext storage of Wi-Fi credentials (CVE-2026-81321). Physical-access flaws allow arbitrary code execution from scripts on removable media (CVE-2026-81305) and unauthenticated bootloader access (CVE-2026-85478). The cameras are deployed worldwide in commercial facilities.

CISA advisory: two hardcoded cryptographic key flaws in Wärtsilä FOS-Onboard 5.07.0923.01 (CVSS 9.0/9.1) could allow unauthorized updates, code execution, and credential theft.

CISA advisory ICSA-26-258-02 discloses CVE-2026-78225 (CVSS 3.1 9.0 critical), a hardcoded cryptographic server key in the deployer-ng Update Controller of Wärtsilä FOS-Onboard 5.07.0923.01. CVE-2026-81855 (CVSS 3.1 9.1 critical) is a hardcoded client authentication key in the robot testing framework component. Successful exploitation could let an attacker deliver unauthorized updates, execute code, or extract credentials to impersonate a privileged client. The product is deployed worldwide in transportation systems critical infrastructure; Cydome Security reported the flaws and CISA states no known public exploitation exists.

Siemens Mendix SAML

Siemens Mendix SAML module fails signature validation, allowing unauthenticated account hijacking in specific SSO configurations (CVE-2026-80465).

CISA republished Siemens advisory SSA-887643 for CVE-2026-80465 (CVSS 8.7) in the Mendix SAML module, affecting Mendix 10/11-compatible versions before 4.2.3 and Mendix 9.24-compatible versions before 3.6.27. The module improperly validates SAML response signatures (CWE-347), letting unauthenticated remote attackers hijack an account in specific SSO configurations. Mendix has released fixed versions and recommends updating.
